If there is currently not a participating dentist close to your home, please send us the names of dentists and dental offices near your home, and we will reach out to them, confirm credentials and good standing, and if appropriate invite them to join the Moonlight Graham dental network.

The Moonlight Graham dental savings club is NOT insurance. This is not a qualified health plan under the Affordable Care Act (ACA). This plan does not meet the minimum creditable coverage requirements under M.G.L. c. 111M and 956 CMR 5.00. This is not a Medicare plan. The plan provides discounts at participating dental care providers for services. The plan does not make payments directly to providers. The member will receive a discount from participating dental care providers, and the member is required to pay for all the dental care services received from the provider. The range of discounts will vary depending on the type of provider and services. The licensed Discount Plan Organization is Moonlight Graham of Wisconsin, LLC. 10201 N.
